What is a CORI Form?
CORI stands for “Criminal Offender Records Information (CORI)” and is a background check mandated by the state for school employees and volunteers who may have direct and unmonitored contact with children. Anyone who wants to volunteer is required to have a current CORI form on file with Holliston Public Schools.
I’m CORI’ed with another organization, do I still need to complete a CORI for the school?
YES. Holliston Public Schools (HPS) must perform CORI checks on all individuals regardless of previous checks by other clubs and organizations.
How do I submit a CORI form?
Go IN PERSON to the Superintendent’s Office at the High School or any school office and ask the office to complete a CORI form.
IMPORTANT: Bring your valid, government-issued picture ID with you as the administrator will need to verify it when completing the CORI form.
You can also print the CORI by clicking the image above, fill it out at home, then bring to the school in person with your ID.
NOTE: The Superintendent’s office is open over the summer - it’s the perfect time to get your CORI done! As you’re facing the High School, the office is the smaller building on the right. The closest parking is located at the front right of the building.
CORI forms can take up to two weeks to process, so get them in well before you intend to volunteer on-site!
How long is my CORI valid for?
CORI checks are valid for three years. To check your CORI status, contact the Superintendent’s office at (508) 429-0654.
